Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (850) 656-7265 during regular business hours.

Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.

Sick Visits - Please call to schedule an appointment before coming to the office. Our physicians have set aside appointments every day for urgent office visits, and we make every effort to see sick patients on the same day. If needed, we will schedule an appointment for a “work-in” to try and better manage patient flow and decrease waiting time.

Routine Appointments - These are for non-urgent, chronic medical conditions, follow-up visits, and medication refills. Please call a week in advance to get the appointment you prefer with your regular healthcare provider. We recommend you make your next follow-up appointment before you leave the office. Also, try to make arrangements to have any lab test completed prior to your visit.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

You can also use our secure Online Bill Pay option to pay current statement balances online without setting up an account.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

We recommend that you call your insurance company to verify that we participate with your plan.

Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.

Patients may be financially responsible for payment of all services even if your insurance company does not pay. Patient accounts that are not paid promptly are subject to third party collections and/or legal action.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

Referrals

Some insurances may require a referral for your visit to our office. Please make sure to contact your insurance company and/or your primary care physician to ensure that you do get a referral if it is required. Failure to obtain a referral, when necessary, may cause your insurance company to deny your claim, and you may have to pay for the visit at the time of service.

If necessary, we will obtain authorization for testing scheduled by our office.

Medical Records

Due to Federal HIPAA Privacy Regulations, a medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.

Refill Requests

We prefer to refill your medications at the time of your office visit. Please bring a complete medication list with you to each appointment.

Please be sure to give us at least 72 hours (3 days) notice on all refill requests.

Chronic or long-term medication refills require an office visit before they may be refilled. If it has been more than 60 days since your last visit, we may not be able to refill your prescription by phone.

After-Hours & Emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (850) 656-7265 and our answering service will assist you.
